Responsibilities

• Assist the Physical Therapist in developing and implementing an individualized care plan.
• Implement the care plan by delivering prescribed exercises and education under the direction of the Physical Therapist.
• Observe, record and report changes in the client's condition and progress to the Physical Therapist and care team.
• Provide education to the client and their family to ensure safety and proper use of equipment and exercises.
• Adhere to all safety regulations and company policies and procedures.
• Document daily progress notes and other required documents accurately and in a timely manner.
• Participate in interdisciplinary team meetings as needed to develop, review, and update care plans.

Requirements

• Physical Therapy Assistant degree from an accredited institution.
• Active State of Michigan PTA license in good standing.
• Minimum of 1 year experience as a Physical Therapist Assistant with home health care experience preferred, but not required.
• Proficient knowledge of therapy administration techniques and equipment.
• Excellent communication and interpersonal skills.
• Ability to work independently, while also serving as an integral part of a team, to manage multiple cases.
• Valid driver's license and reliable transportation.
