An exciting and rewarding opportunity has arisen within the Acute Peripatetic Team.
The role offers a diverse and varied workload, operating within a busy environment. The post holder will be fully engaged in supporting our Acute Adult Social Care colleagues across Central, East, North and West Lancashire in line with the Hospital Discharge Policy.
Lancashire Adult Social Care has transformed how we assess, deliver, and support our citizens in Lancashire with a significant focus on promoting and maintaining independence through creative support planning for our diverse communities in Lancashire. This is a far more efficient way to manage and run the peripatetic team as it offers consistency and continuity to the current established team.
The Acute Peripatetic Team utilises the Discharge to Assess pathway and champions the Home First ethos. The team strives to maintain patient flow and to provide better lives for the people of Lancashire. The post holder will be supporting people to return home, including planning complex discharges and reducing delays/long length of stay in hospital.
The post holder will become an integral part of a countywide team, working in partnership with Lancashire County Council and the NHS. We are a fluid and integrated resource that assists in relieving pressures within both ASC Acute Services in partnership with our health colleagues, Age UK, Housing colleagues and carers services.
This role will include working in a person centred and strength-based manner. The post holder will complete hospital discharge planning and triage, review care packages and care home placements and provide cover and assistance to the locality hospital teams as part of a multi-disciplinary team. The post holder will be required to cover weekends on a rota basis will cover for locality team weekend staff when they take annual leave.
The successful candidate will be an expert in identifying the right level of care at the right time and have the ability to use their professional judgment and initiative to ensure individuals are provided with the tools to achieve their optimum level of independence.
It’s a full-time post (37 hours) with enhanced rates of pay for hours worked at weekends and Bank Holidays. Weekend work is arranged on a rota basis with an expectation that a successful candidate will work up to 18 weekends in a 12-month period.

This is a car essential user post. Mileage across Lancashire will be calculated from County Hall, Preston.

Location: Countywide. Base is County Hall.
Service: Adult Social Care – Acute Peripatetic Team
Contract Type: Permanent– Working pattern will be over 7 days (up to 26 weekends per year), including bank holidays. The Acute Peripatetic Team currently operate from 8am – 6pm but in the future will be moving to an 8am – 8pm working pattern.
