Job Descrption
Manages, designs and implements bereavement program and services including workshops and support group sessions. Provides support and counseling to patients and families dealing with grief. Initiates bereavement contact, develops plans of care in conjunction with the interdisciplinary team, and documents ongoing care. Serves as a liaison between families, the hospital, and other community agencies and resources. Facilitates the recruitment, training, and supervision of bereavement volunteers. Develops and tracks statistical information regarding utilization of services, and program outcomes.
Bachelor's Degree in Social Work, Counseling, or other related field required. Master's Degree preferred. Applicable Indiana license required.
Requires 0-1 years of relevant experience.
Requires strong clinical assessment, planning and goal development skills.
Requires strong counseling skills to help patients and families understand, accept and follow medical recommendations.
