A Clinical Liaison serves as an advocate for patients during the pre-admission process. You will drive to hospitals in your assigned territory to meet with discharge planners and assist with assessing and transferring patients who qualify for long term acute care services. You will serve as a resource for the hospital and its' staff and develop collaborative relationships with doctors and case managers. You will become a subject matter expert in the conditions that qualify patients for care in our hospital, and you will navigate insurance and other factors that can interfere with access to care.
Select Specialty Hospitals offer a post-ICU plan of care that focuses on mobility, weight-bearing, ambulation and nutrition - all crucial contributors to getting patients back to their baseline of health. Clinical Liaisons help hospitals in their local communities to identify post-ICU patients who have entered into a state of chronic critical illness and will benefit from this specialized extension of acute care.

This is a salaried position that works Monday-Friday 8 AM - 5 PM with periodic on-call responsibility.
