Position: Travel Registered Nurse (RN) Home Health/Hospice
Registered Nurse
This position will function as part of our start-up and survival team (S&S). As we expand our footprint, presentation, articulation, and a strong skill set are crucial for introducing our brand. Team members will manage the initial setup and transition to the permanent clinical team through onboarding and training. They will be or will become Matrix Care power users. Our team ensures seamless operations during growth, extended PTO, or other gaps.
These team members prevent extra strain on the team, keeping patient care our top priority. This position will require a person to travel to different states and sites on a weekly basis.
Responsibilities:
• Deliver compassionate and comprehensive nursing care to patients facing end-of-life challenges.
• Provides skilled nursing care including administration of medications and treatments
as prescribed in the physician’s plan of care for patients in their home... setting.
• Conduct thorough assessments to identify patient needs and develop individualized care plans.
• Manage medications and treatments as prescribed by the physician.
• Provide emotional support and guidance to patients and their families.
• Educate patients and families about hospice care and end-of-life issues.
• Collaborate effectively with interdisciplinary team members to ensure coordinated care.
• Maintain accurate and up-to-date patient records.
• Ability to travel up to 100% of the time.
• Perform other duties as assigned.
Qualifications:
• Current and active RN license in the state of employment.
• We are seeking individuals who either hold an RN license or are willing to obtain one in the following states: CT, NY, MA, NH, ME, TX, OH, NJ, PA, and VA.
• Bachelor’s degree in nursing (BSN) preferred.
• Minimum of 1 year of experience in hospice or home health.
• In-depth understanding of end-of-life care principles and practices and certified home health.
• Exceptional communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and empathetic approach to patient care.
